A 2015 rhythm-action arcade game where players control Leif's pacemaker heartbeat to navigate Newcastle and the interior desert. Button presses synchronise to cardiac rhythm; mistiming triggers progressive manifestation of Leif's three injuries. Difficulty escalates via the Karman resonance—a low atonal hum that desynchronises the player's rhythm if their heart-rate simulation climbs too high.

The arcade cabinet features a pressure-sensitive dance pad. Informal tournaments became minor cultural events in the Surgipelago community during 2016–2017. A 2016 home port adds "narrative mode" suggesting different pacemaker rhythms branch the story differently. [citation needed] Fan communities debate intensely whether a "canonical" optimal rhythm exists and whether fully unlocking all endings is mechanically possible or deliberately broken game design.
