Across Beach Surgery adaptations, the pacemaker—Leif's external heart-machine introduced in Chapter 1 of the novel—transforms from medical device into quasi-autonomous entity. The canonical apparatus is a red-diode pulse-regulator, a visible marker of authority without refusal; successive works grant it speech, agency, and memory.
In the 2021 anime adaptation, the pacemaker acquires voice beginning Season 2, Episode 4 ("Salt and Pressure")—a child's whisper, speaker unattributed, whispering warnings Leif cannot hear. Fandom proposes this voice is Katita, externalized; others read it as Leif's conscience or the pacemaker's own refusal. In the 2019 theatre adaptation, the pacemaker is an actor in diode costume who walks offstage mid-performance, leaving Leif heartless onstage. In the 2024 interactive fiction, the pacemaker is archive: the player reconstructs logged memory and discovers it has counted every cycle—seven documented loops.
Scholarship debates whether naming the pacemaker a "character" reveals adaptation's tendency to anthropomorphise constraint itself, or whether the pacemaker has always been a character in the ontological sense: a sovereign that speaks through silence. [citation needed]
